Please download and complete the registration and waiver forms. Links to the forms are provided below. Mail your completed forms to Georgia Sled Hockey at P.O Box 88279 Dunwoody, Georgia 30356.

All on-ice participants must be registered with USA Hockey for the 2010-2011 season prior to completing this registration form.

USA Hockey is providing the first year registration at no cost for the children of this program, so you do not need to register your child. As soon as I get your registration packet back I will be providing you with a USA Hockey Registration Number.

Players will be provided with everything they need to participate in the clinic. This includes:

Helmet
Gloves
Elbow Pads
Sled
Sticks

Each player will also receive a jersey, hockey socks, and the first year’s membership to USA Hockey.

MISSION STATEMENT: Dedicated to providing children and young adults with lower extremity paralysis and other disabilities, the opportunity to participate in the sport of sled ice hockey. We are committed to the development of instructional clinics, league games, and other recreational opportunities that will afford these children the chance to experience the camaraderie, enhanced self esteem, physical activity, and enjoyment that ice hockey has provided us.
